The organization is a leader in advancing medical education and research, with a robust pharmacy residency program that includes both PGY1 and specialized PGY2 programs in critical care, infectious diseases, and emergency medicine.

This role is a direct result of the organization's growth, spurred by the expansion of its hospitalist services and the establishment of a new transplant center. Joining this team means becoming part of an integrated, multidisciplinary environment where pharmacists are highly respected and actively involved in all aspects of patient care, contributing to protocols and educational initiatives.

Job Description :

Our client is seeking a dedicated and highly proficient Clinical Pharmacy Specialist to focus on solid organ transplantation, with a primary emphasis on kidney transplant patients. This is a direct patient care role that encompasses the entire continuum of care, from initial patient selection through long-term post-discharge follow-up. You will play a pivotal role in optimizing medication regimens, particularly immunosuppressive drugs, to ensure long-term allograft preservation. Your responsibilities will also include managing treatments for co-morbidities and providing comprehensive medication education to patients and their families during critical transitions of care.

A key part of the position involves participating in multidisciplinary rounds on the inpatient service, where you will provide your expert clinical judgment. You will facilitate the appropriate selection of treatments for cases of acute or chronic allograft injury or rejection, leveraging established protocols. The role also includes teaching and mentorship opportunities, as you will serve as a preceptor for pharmacy students and residents, fostering the next generation of pharmacy leaders. Beyond patient care, there are ample chances to contribute to longitudinal projects, including research, medication use evaluations, and process improvement initiatives. The institution boasts a robust staffing model and advanced technological systems designed to enhance medication safety.

Must Haves:

  • Graduation from a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE)
  • A Virginia pharmacist license or eligibility for licensure is required.
  • ASHP-accredited PGY1 Pharmacy Residency
  • Minimum of three years of previous clinical pharmacy experience in a relevant care setting.
  • Exceptional leadership, communication, interpersonal, and teamwork skills.
  • Demonstrated advanced skills in critical thinking and analysis.

Preferred Skills:

  • Completion of a specialized PGY2 residency in Solid Organ Transplantation.
  • Prior experience with high-acuity, intensive care unit-level patients.
  • Expertise in managing and adjusting medication dosing protocols for specific drugs, such as vancomycin, heparin, warfarin, and various antimicrobials with renal dosing.
  • Experience with implementing and leveraging new pharmacy systems or technologies to enhance medication safety.
  • Experience providing lectures or educational sessions within a medical school setting.
